• Utility Bill for proof of residency

• Proof of Insurance or Insurance Waiver

• Physical-this is the only form not required at registration.  It will be required before the 1st day of practice (No Exceptions!)

• Grade verification from school the player is enrolled in

• Please attach required docs to the online registration form
